Our Community Care program is designed to help patients who are uninsured or underinsured, as well as those who may be experiencing financial hardship. See the program summary for more detailed information (English | Spanish).

You will also need to include the below documentation with your application. (Patient Financial Counselors may require other verification of income or assets when deemed necessary.)

• Copies of the two most recent pay stubs for all adult family members
• A copy of your most recent federal tax return
• A copy of unemployment or pension compensation

Applicants will be notified in writing of approval or denial within 14 days of receipt of a complete Community Care application.
